IMPORTANT: Varsity Participation Paperwork
Parents of Football Players Grades 10-12:
Since our young men sometimes don't communicate all the information we ask them to share with their parents, I am going to
give you some VERY important paperwork information that must be completed and turned in to Coach Kenedy by August 7th or
your son will not be able to practice with the team. Every athlete has something that MUST be filled out!
1.Your son must have a current physical on file with trainer Stephen Moss. We have shared with the team which players don't yet
have a physical on file. Many took their physical in the spring, and that is fine. If they have not had one, you may go to urgent
care or your family Dr. and take a copy to Stephen Moss by August 7th.
2.You must also print off and turn in the parental consent form, and drug contract and turn in a HARD copy.
3.Every other form may be completed online and I am attaching all instructions on the PDF form.
4.Drug Test. Every athlete will take a mandatory drug test and IMPACT concussion test Wednesday August 7th after workouts.
The Drug test is $25.00. Athletes must have this to practice.
- Wednesday August 7th- Drug Test and IMPACT tests. 9AM
- Monday August 12th- Practice Begins (7-9am, 10-11:30am) The OSSAA has allowed us to begin practice a day earlier than in
years past. We will begin our two a days Monday August 12 and meet with the team between practice sessions.
- Saturday August 17th- Team Pictures 7AM, Parent Meeting 9AM, Scrimmage 9:30AM.
We will relay more important dates to you as they come, but please be diligent and help your child get the appropriate paperwork
